Pretty much, to get judas (in my opinion, the best character) you have to kill satan for the first time. You can find satan in sheol. Sheol is the floor after the mom's heart/ it lives fight.
If you are far enough in the game, there will be two entrances, down and up. In rebirth, judas is unlocked by defeating satan for the first time with any character. He is unlocked by completing the womb or utero for the first time.

I unlocked sheol on my first time beating the heart. But i took the chest for eden that time and it took 6 more defeats of the heart. The lost is one of the trickiest characters to unlock in the game.
